All our team works hard to ensure we’re providing you and your loved ones the best possible care and attention. We currently employ 40 staff consisting of Nurses, Assistant Practitioners, Health Care Assistants, and an Administration team. This staff is led by a Clinical Lead Manager,  an Operational Business Manager and a District Nurse Practice Educator.

MRCGP, DRCoG, CEO of Freshney Pelham Care Ltd and GP Partner for Littlefield Surgery. Dr Dukes qualified as a doctor in Amsterdam and as a GP in NE Lincolnshire in 2007 and is also a GP and HMYS medical student trainer and GP appraiser.

SENRGN Dip N Bsc(Hons) District Nurse Specialist Practitioner is responsible for the clinical leadership of the team and also has the role of Preceptor Lead, Education Lead, Safeguarding Lead, Link Practitioner, Mentor and NMC revalidation registered confirmer.

Operational Business Manager
Extensive Business and Management qualifications; Joined the team in January 2019 and responsible for all aspects of Business and Finance including HR, H&S, DP Lead, and contracts.

Mrs Janet Lee

District Nurse Practice Educator
SRN, RMN, District Nurse and Nurse Tutor; Associate member of the Association of District Nurse Educators. Obtained the Queen’s Nurse Title in 2016 in recognition for contribution to high standards of patient care and continually improving practice’.Responsible for all aspects of staff development and education within community nursing and working with Clinical Lead to implement learning and changes to enhance quality service delivery.

Emily Dixon

Admin Supervisor.
Emily as worked within Freshney Green for over 10 years working her way from Administration Apprentice to Administration Supervisor. Emily is responsible for the day-to-day administration duties of the team and staff.

Olivia Barton

Administration Assistant.
Olivia joined us a year ago on an apprenticeship programme. Olivia is now responsible for general administration duties, the website and social media.

Eleanor has recently joined the FPC as an Admin Assistant and is responsible for general administration duties.

District Nurse Operational Team Leader

I have been working with FPC for 6 years starting as a newly qualified nurse. In this time I have had the opportunity to further my career gaining qualifications in Mentorship and Nurse Prescribing. Most recently I have completed the Specialist Practitioner Qualification in District Nursing enabling me to manage and support the Fieldhouse Team.

Assistant Practitioner
I have worked in healthcare for 8 years within hospital and community settings. I am now an Assistant Practitioner for FPC and am looking forward to further opportunities and challenges.

Hollie Gray

Health Care Assistant
I am currently a HCA within the community nursing team and have been so for the last 5 months. I am able to provide wound care, obtain blood, take observations, monitor INR levels and more. I am soon to start a medicine management  course and hoping to progress my career within FPC in the future.

DN Team Leader for the Pelham practice community nursing team. Qualified as nurse in 2003 with 11 years’ experience in vascular surgery, critical care, colorectal and breast surgery before moving out into primary care 6 years ago. I have completed my DN Specialist Practitioner Qualification, am a sign off mentor and community nurse prescribe

Andrea Birdsall

Deputy Team Leader
Qualified with a BSc in Adult Nursing, obtained a Diploma in Diabetes and a Community Nurse Prescriber qualification and Mentorship Programme.  Is a Diabetic Link Nurse, Tissue Viability Link Nurse and mentor for students.

I qualified as a registered nurse in 2003, I have worked for FPC since December 2018, I am a student mentor, and my daily responsibilities include phlebotomy, b12, flu vaccination, wound care, catheter, bowel, palliative and end of life care.

Sophie Cooke

I have been qualified since November 2019, and am working with FPC in my first nursing position. I love nursing patients in the community for the vast variety of patients and conditions that we see.

My name is Ellie Clarke. I am the long term conditions team leader. I have 18 years’ experience working as community staff nurse, senior nurse in the Haven Team and finally working with complex health and social needs with patients with a long term condition, preventing any unnecessary hospital admissions. My passion is palliative and end of life care.

Joanne Dean

I have been a registered nurse for almost ten years and have worked within both hospital and community settings. I have worked for FPC for just over 2 years and I am currently working as a community nurse within the Long Term Conditions team. I am enjoying this role and am undertaking a module in Supporting Adults with Long Term Conditions at the University of Hull.

Senior Staff Nurse
Nicola has 8 years community nursing experience across Lincolnshire. She has special interest in wound healing, and is a tissue viability link nurse for the team.
