Five GP practices including: Fieldhouse, Humberview, Pelham, Woodford and Littlefield

We cover around 40,000 patients across the Grimsby and Cleethorpes area. Each surgery is represented on our Board of Directors by a nominated GP. There are currently 40 staff consisting of Nurses, Assistant Practitioners, Health Care Assistants, and an Administration team.

These staff are led by a Clinical Lead Manager, an Operational Business Manager and our District Nurse Practice Educator. We are  unique in that we have a Practice Educator to support our staff in their continued professional development.

We are based within the Freshney Green Primary Care Centre on Sorrell Road, Grimsby, and this is a modern purpose built centre which in addition to 3 GP practices houses the adult social care team, an NHS dentist, Mental Health Services, as well as physiotherapy, audiology and chiropody services, Minds Matter and the NSPCC.

As a provider of health care, our commitment is to deliver services to the population which include nursing care, support and advice, signposting, and preventative services to enable independence for as long as possible.

Integrated approach to nursing

We work with an integrated approach to nursing and liaise with GP’s, social care colleagues and other health care providers. It is our holistic approach which promotes and supports independence.

Our Aims:

  • To provide care that is effective and efficient
  • To provide high-quality nursing care within the community for housebound patients
  • To provide services which will enable our patients to maintain their independence for as long as possible
  • To provide support for those with more complex needs and long term conditions
  • To work closely in partnership with other providers and partners to improve outcomes for our patients

 

Our objectives

  • To deliver care across all areas of the community, based on need and delivered in a safe manner with effective systems and processes in place
  • To support the patients, promote health and independent living and enable patients to remain in their own home as long as possible
  • To work closely with partners and agencies
  • To ensure effective communication and decision making processes are open and transparent between patients, carers, families and their representatives
  • To promote and provide advocacy services

A big award winner on our amazing team

A local triumph was achieved by North-East Lincolnshire’s district nurse team Freshney Pelham Care, when their clinical lead manager nurse Elizabeth Clift achieved the ‘highly commended’ accolade in the National Good Practice award for Clinician of the Year 2019. She was the only shortlisted nurse (and up against GPs) in her category, which was announced at the prize-giving ceremony at Birmingham’s NEC.
