In line with NHS England’s ‘Framework for Enhanced Health Care in Care Homes March 2020’, our Care Home Team runs alongside the Long Term Conditions and Community Nursing Team for all residents of our PCN’s aligned care homes.

The aligned care homes are:

• Eaton Court
• St Margarets
• Yarborough House
• Holmefield House
• Eastwood House

FPC provides nursing care for all patients currently registered with the GP practices aligned to the PCN, and liaises with other service providers for the benefit of those patients.

As further patients within the aligned care home register with the PCN practices, our Team Leader’s will ensure that each patient has a holistic assessment completed and personalised care plan discussed with them. A weekly ‘home round’ to review patient needs supports the weekly multi-disciplinary discussions ensuring the most up to date and relevant information is available in relation to the patient’s care.

Our Team also provides guidance, support and education to the staff working in the aligned care homes and is an integral part of the service provided by our team.

The aim of this service is to ensure the patient will continue to receive timely care and that effective collaboration with all professionals is provided.
